Niet-klinische factoren die het voorschrijven van antibiotica door dierenartsen bepalen: een systematische review
Het misbruik van antibiotica bij mensen, dieren en planten houdt verband met de verspreiding van resistente antibioticumstammen onder mens en dier. In dit artikel voeren we een bibliografische zoekopdracht uit in Medline, Web of Knowledge en Cab Abstracts met als hoofddoel het beschikbare bewijsmateriaal vast te stellen over niet-klinische factoren en attitudes die het voorschrijven van antibiotica door dierenartsen kunnen beïnvloeden. In totaal voldeden 34 onderzoeken aan de inclusiecriteria. Terwijl de voorschrijfgewoonten van veterinaire gezondheidswerkers niet leken te worden beïnvloed door hun sociaal-demografische kenmerken, werden ze beïnvloed door verschillende attitudes, zoals angst (geïdentificeerd in 19 van de 34 onderzoeken)
zelfvertrouwen (19/34), zakelijke factoren (19/34), en door zelfgenoegzaamheid (16/34). Bepaalde eigenaarsgerelateerde factoren, zoals een gebrek aan bewustzijn (16/34) en de vraag naar antibiotica (12/34), waren ook belangrijk, evenals gelijktijdige factoren, variërend van een gebrek aan passende regelgeving (10/34) tot de kosten en vertragingen bij het uitvoeren van kweek- en gevoeligheidstests (10/34) en inadequate bedrijfshygiëne (8/34). Onze resultaten lijken erop te wijzen dat de niet-klinische factoren mogelijk kunnen worden gewijzigd.
Dit kan nuttig zijn voor het ontwerpen van interventies gericht op het verbeteren van het antibioticagebruik bij dieren, als onderdeel van een algemene strategie om de wereldwijde verspreiding van multiresistente stammen te verminderen. Het doel van dit werk is om bij te dragen tot de beoordeling van multi-residu-analyse van geneesmiddelen voor diergeneeskundig en humaan gebruik met behulp van UHPLC-QTOF in urine en bloed van vee (runderen, kippen, schapen en varkens). Ten eerste werd een interne database met samengestelde naam, mono-isotopische massa, chemische formule, retentietijd, chemische structuur en drie CID MS-MS-spectra van de 234 geselecteerde geneesmiddelen gebouwd voor kwalitatieve detectie.
Een onderzoek naar de perceptie van dierenartsen ten aanzien van het welzijn van kalveren in Nieuw-Zeeland
Ondanks recente wetswijzigingen om gebieden aan te pakken met het grootste risico voor het welzijn van kalveren (Bos taurus) in Nieuw-Zeeland, bestaat de bezorgdheid dat wetenschappelijke kennis over dierenwelzijn in de praktijk niet is overgenomen. Als onderdeel van een grotere, landelijke studie waarin de perceptie van dierenartsen ten aanzien van het welzijn van kalveren werd onderzocht, was het doel van het huidige werk om de perceptie van dierenartsen ten aanzien van het niveau van welzijnsbescherming dat wordt geboden aan jonge “bobby” -kalveren in Nieuw-Zeeland, te onderzoeken. Deze studie onderzocht ook de bezorgdheid over welzijnsschendingen en identificeerde belemmeringen voor welzijnsgerelateerde veranderingen voor kalveren in het algemeen.
Een elektronische enquête met gemengde methoden werd ingevuld door 104 dierenartsen die geregistreerd waren bij de Veterinary Council of New Zealand. Uit de bevindingen bleek dat dierenartsen het sterk oneens waren met de specificaties van bepaalde welzijnsvoorschriften voor kalveren. Dierenartsen identificeerden ook gebieden met het grootste risico op schending van het welzijn van kalveren in de productieketen en belemmeringen voor welzijnsgerelateerde veranderingen. Deze bevindingen tonen aan dat dierenartsen aanzienlijke steun hebben voor het verbeteren van het niveau van welzijnsbescherming van kalveren.
Gezien de discrepanties die bestaan tussen het huidige reguleringsregime en veterinaire perspectieven, kan de opgedane kennis uit deze studie worden gebruikt ter ondersteuning van hervorming van de regelgeving om het welzijn van kalveren in de praktijk en het beleid in Nieuw-Zeeland te versterken. Clostridioides difficile wordt vaak aangetroffen bij dieren en hun omgeving. Er is echter niet veel gerapporteerd over de omgeving van dierenklinieken in termen van de sporenbelasting, prevalentie en PCR-ribotypediversiteit. Het doel van deze studie was om de prevalentie van C. difficile op schoenzolen van dierenartsen, veterinair ondersteunend personeel en veterinaire studenten op de campus van de Veterinaire Faculteit te beoordelen.
In totaal werden 50 wattenstaafjes voor schoenzolen verzameld, de positiviteitspercentages varieerden van 86,7% in wattenstaafjes van dierenartsen tot 100% in wattenstaafjes van ondersteunend personeel en studenten. Niet-toxische en toxigene stammen die toxinotype 0, IV en XIX vertegenwoordigen, werden geïsoleerd en verdeeld over 17 verschillende PCR-ribotypen, waarvan de meest voorkomende 010, 014/020, SLO002 en 009 waren. Ten tweede toonde het validatieresultaat van de methode aan dat alle 234 geneesmiddelen een goede lineariteit vertoonden met determinatiecoëfficiënten (R2) hoger dan 0,999.
One Health Integration: een voorgesteld kader voor een onderzoek naar dierenartsen en het beheer van zoönosen in Ghana.
Parallel met de recente wereldwijde promotie van One Health (OH) als beleidsconcept, roept een groeiend aantal sociaalwetenschappelijke studies vragen op over hoe succesvol OH-beleid en -programma’s zijn geweest bij het beheersen van sommige wereldwijde gezondheidsproblemen, zoals zoönotische ziekten. . In dit artikel wordt deze literatuur kort besproken om het kritische perspectief ervan te verduidelijken. Veel van de literatuur over OH is ook gericht op gezondheidsmanagement op internationaal niveau en heeft minder aandacht besteed aan implementatieprogramma’s en beleid voor OH op nationaal en lokaal niveau, vooral in lage- en middeninkomenslanden (LMIC’s).
Programma’s om OH te implementeren zijn vaak gekoppeld aan het concept van “integratie”, een begrip dat geen universele definitie heeft, maar niettemin een centraal uitgangspunt en doel is in veel OH-programma’s. Op lokaal en nationaal niveau kunnen sterke verschillen in perspectieven over OH tussen verschillende beroepen een grote belemmering vormen voor de integratie van die beroepen in de implementatie van OH. Beleid dat is gebaseerd op integratie tussen beroepen in sectoren als dier-, mens- en milieugezondheid kan de identiteit van beroepen bedreigen en kan dus op weerstand stuiten.
Rekening houdend met deze kritiek op OH-onderzoek en -implementatie, stelt dit artikel een onderzoekskader voor om de dominante sociale dimensies en machtsdynamiek onder professionele deelnemers te onderzoeken die invloed hebben op OH-implementatieprogramma’s op lokaal en nationaal niveau in een land met lage inkomens. De voorgestelde onderzoeksfocus is het beroep van dierenarts en een aspect van OH waarin dierenartsen noodzakelijke actoren zijn: zoönotische ziektemanagement. Resultaten van onderzoek dat op deze manier wordt ingekaderd, kunnen onmiddellijk worden toegepast op de programma’s die worden bestudeerd en kunnen informatie geven over uitgebreider onderzoek naar de sociale determinanten van succesvolle implementatie van OH-programma’s en -beleid
Onderzoek naar de factoren die het geluk van Zuid-Afrikaanse dierenartsen beïnvloeden
Geluk is een nieuw studiegebied op verschillende gebieden, waaronder gezondheidszorg en diergeneeskunde. Werkgeluk, of subjectief welzijn in de werkomgeving, is een prominent onderzoeksgebied geworden. Het geluk van dierenartsen heeft de afgelopen jaren wereldwijd academische belangstelling gekregen. Eerder onderzoek gaf aan dat een verhoogd geluksniveau van werknemers sociaal, persoonlijk en mogelijk financieel gewin oplevert voor werkgevers en werknemers. De doelstellingen van deze studie waren om de factoren te bepalen die het geluk van Zuid-Afrikaanse dierenartsen beïnvloeden en om een conceptueel model te ontwikkelen op basis van de geïdentificeerde factoren.
Een cross-sectioneel onderzoek met behulp van een kwantitatief onderzoek werd uitgevoerd met behulp van een gestandaardiseerde vragenlijst. Van de 2.182 geregistreerde dierenartsen vulden 360 praktiserende dierenartsen de enquête in en de resultaten werden statistisch geanalyseerd met behulp van verkennende factoranalyse. De resultaten gaven aan dat de factoren die van invloed zijn op de werkplek, sociale relaties, tevredenheid met de balans tussen werk en privé, doel, optimisme, werkvoldoening, werkstress en vrije tijd werden geïdentificeerd als significante statistische relaties met het geluk van dierenartsen.
Op basis van de onderzoeksresultaten worden managementadviezen gegeven. Deze studie is de eerste bekende studie om de factoren te onderzoeken die het geluksniveau van dierenartsen beïnvloeden. Het onderzoek vormt de basis voor soortgelijk onderzoek dat in andere landen wordt uitgevoerd. Volgens 66,9% van de respondenten was de handenwasgelegenheid vaak toereikend op veehouderijen, maar gaf slechts 21,4% aan dat dit het geval was in stallen (p <.001). We beschrijven hoe RVC een IPE-team heeft ontwikkeld dat bestaat uit faculteitsleden die voorstander zijn van IPE, wat op zijn beurt studenten heeft geïnspireerd om een door studenten geleide IPE-club op te richten die buitenschoolse educatieve evenementen organiseert.
Terwijl 75,0% aangaf dat ze hun handen moesten wassen of handdesinfecterend middel moesten gebruiken voordat ze naar de volgende boerderij gingen, gaf slechts 42,5% aan dit te hebben gedaan voordat ze naar de volgende stal gingen (p <.001). Universele beschermende vacht of overall werd vaker gebruikt in de veehouderij dan in de paardenpraktijk (91,6% vs. 27,7%, p <0,001). Stethoscoopreiniging werd door 30,0% van de respondenten gemeld minder vaak dan eens per week plaats te vinden. In totaal hebben 61 dierenartsen de vragenlijst teruggestuurd.
[Onderzoek naar het gebruik van antibiotica bij exotische huisdieren onder Zwitserse dierenartsen]
Om het verstandig gebruik van antibiotica de afgelopen jaren te bevorderen, zijn wereldwijd monitoringprogramma’s en richtlijnen ontwikkeld over het gebruik van antibiotica en antibioticaresistentie in de humane en diergeneeskunde. Dergelijke informatie over het gebruik van antibiotica bij exotische huisdieren ontbreekt echter. Hiermee moet rekening worden gehouden, aangezien het aantal exotische huisdierenpatiënten constant toeneemt en de toediening van antibiotica bijzonder uitdagend is vanwege de diversiteit aan soorten en de verschillen in fysiologie.
De huidige studie rapporteert de resultaten van een onderzoek naar veelgebruikte antibiotica en de criteria voor antibioticagebruik bij exotische huisdieren (konijnen, knaagdieren, vogels en reptielen) onder Zwitserse dierenartsen. Deze gegevens zouden de basis moeten vormen voor de ontwikkeling van richtlijnen voor antibioticagebruik bij exotische huisdieren. De belangrijkste beslissingscriteria voor antibioticabehandeling en antibiotica-selectie waren klinische symptomen (55/59, 93% voor antibioticabehandeling en 40/59, 68% voor antibiotische selectie), ervaring (41/59, 69% en 36/59 , Respectievelijk 61%) en studieboeken (39/59, 66% en 40/59, 68%).
De belangrijkste beslissingscriteria voor dosering en therapieduur waren studieboeken (59/59, 100%) en ervaring (31/59, 53%). Het gebruik van een microbiële cultuur werd door 39% van de deelnemers als beslissingscriterium gebruikt voor zowel antibioticabehandeling als antibiotica-selectie, gevoeligheidstesten werden gekozen als beslissingscriterium door 37% voor antibioticabehandeling en door 46% voor antibiotica-selectie. Er zijn een aantal uitdagingen bij het verankeren van IPE, maar de voordelen die het biedt bij het integreren van klinische en professionele elementen van de curricula, maken het het overwegen waard.
Fluoroquinolonen werden het meest gebruikt, 46/56 (82% van de deelnemers) voor konijnen, 49/57 (86%) voor knaagdieren, 36/37 (97%) voor reptielen en 38/46 (83%) voor vogels. De overgrote meerderheid van de dierenartsen (57/58, 98%) zou een gids voor het gebruik van antibiotica raadplegen. Het veelvuldig gebruik van kritische antibiotica bij exotische huisdieren onderstreept de behoefte aan een handleiding voor verstandig gebruik van antibiotica. De positieve invloed van dergelijke richtlijnen is al bewezen bij honden en katten. Dit is een voorbeeld van een effectief partnerschap tussen leerling en leraar.
Ontwikkeling van interprofessionele onderwijsinitiatieven ter ondersteuning van het werken en leren tussen dierenartsen en dierenartsverpleegkundigen / dierenartstechnici
De veterinaire werkplaats bestaat uit verschillende professionals die samenwerken in interprofessionele teams. Eerder werk heeft de voordelen onderzocht van effectief veterinair teamwerk voor meerdere belanghebbenden. In dit artikel over onderwijstips schetsen we de onderliggende onderwijstheorieën en tips voor het ontwikkelen van interprofessioneel onderwijs om de waardering van leerlingen voor de verschillende rollen en verantwoordelijkheden van dierenartsen en dierenartsverpleegkundigen / dierenartstechnici te bevorderen. Interprofessioneel onderwijs (IPE) vereist dat studenten met, over en van elkaar leren en impliceert de erkenning van sociaal leren als een ondersteunende benadering.

Studenten krijgen de kans om de complexiteit van interprofessioneel teamwerk in een veilige omgeving te verkennen met behulp van real-life onderwerpen als context voor hun samenwerking. Twee veterinaire voorbeelden van IPE aan het Royal Veterinary College (RVC) worden gegeven om verschillende lesmethoden en onderwerpen te onderzoeken die in onze context succesvol zijn gebleken: tandheelkunde en scenario’s voor gericht leren.
Gedragsgezondheid en slaapproblemen bij dierenartsen van het Amerikaanse leger en veterinaire technici die deelnemen aan de Millennium Cohort Study
Dierenartsen hadden een hogere prevalentie van psychische problemen, slaapproblemen en gebrek aan sociale ondersteuning dan niet-traumadokters, traumadokters of tandartsen. Bij multivariabele analyse hadden dierenartsen een grotere kans op psychische problemen, slaapproblemen en gebrek aan sociale steun, vergeleken met artsen en tandartsen samen; De kans op deze uitkomsten was ook hoger voor dierenartsen, vergeleken met verschillende individuele referentiegroepen. Veterinaire technici hadden een lagere prevalentie en een lagere kans op gebruik van psychotrope medicatie, vergeleken met medici. Om de prevalentie en relatieve kans op psychische problemen, zelfmoordgedachten, gebruik van psychotrope medicatie, drankprobleem,
slaapproblemen en gebrek aan sociale steun onder dierenartsen en veterinaire technici te bepalen, in vergelijking met andere medische professionals, in het Amerikaanse leger. Deze studie werd uitgevoerd in twee regio’s in Spanje (Catalonië en Galicië) via acht focusgroepen; vier voor melkveehouders en vier voor dierenartsen. De resultaten toonden aan dat melkveehouders en dierenartsen elkaar de verantwoordelijkheid toeschrijven voor het niet naleven van bioveiligheidspraktijken. Het onderzoek brengt tegenstrijdigheden aan het licht tussen dierenartsen en bepaalde individuele dierenartspraktijken die aan het onderzoek hebben meegewerkt, wat leidt tot twijfel en verwarring bij melkveehouders.
Er werden ook duidelijke percepties geïdentificeerd over de rol die overheden zouden moeten spelen met betrekking tot zowel opleiding als sancties als middel om de bioveiligheid op melkveebedrijven te verbeteren. Bovendien hadden de deelnemers verschillende meningen over de vraag of biobeveiligingsmaatregelen verplicht moeten worden gemaakt of vrijwillig moeten blijven. De resultaten van deze studie onderstrepen de noodzaak om initiatieven te bevorderen waarmee verschillende belanghebbenden, zoals dierenartsen, overheidsinstanties en melkveehouders, op consensus gebaseerde boodschappen kunnen ontwikkelen over de implementatie van bioveiligheidspraktijken.
Factoren die verband houden met de bezoekspecifieke tevredenheid van voedseldierenproducenten na interactie op de boerderij met een dierenarts
Effectieve communicatie blijkt positieve associaties te hebben met klanttevredenheid, klanttevredenheid en dierenartstevredenheid in de geneeskunde voor gezelschapsdieren. Een beter begrip van de rol van communicatie met betrekking tot deze resultaten, met name de tevredenheid van de producent, is waarschijnlijk gunstig voor de diergeneeskunde. De doelstellingen waren het identificeren van factoren die verband houden met de specifieke tevredenheid van het producentenbezoek na interacties tussen dierenarts en producent op de boerderij en het evalueren van de validiteit van de klanttevredenheidsvragenlijst voor gebruik in de geneeskunde van voedseldieren.
Een cross-sectionele steekproef van praktiserende voedseldierenartsen en hun producerende klanten werd gerekruteerd in Ontario, Canada. Direct na de interactie tussen dierenarts en producent vulden producenten een vragenlijst in die hun bezoekspecifieke tevredenheid en hun perceptie van de producentgerichtheid van de interactie beoordeelden. Dierenartsen vulden een vragenlijst in waarin hun perceptie van de producentgerichtheid van de interactie werd beoordeeld. Er werd een lineair regressiemodel ontwikkeld om factoren te identificeren die verband houden met de specifieke tevredenheid van het producentenbezoek.
Eenenveertig dierenartsen en 207 producenten namen deel. Factoren die verband hielden met de specifieke tevredenheid van producentenbezoeken waren onder meer de leeftijd van de producent (tevredenheid daalde met de leeftijd), het geslacht van de producent (mannen minder tevreden) en de perceptie van de producent van de score voor producentgerichtheid (de tevredenheid nam toe met de perceptie van de producent over de score voor producentgerichtheid). Bevindingen vergroten het bewustzijn van het belang van de perceptie van producenten over producentgerichtheid in de praktijk van voedseldieren en moedigen het gebruik ervan door dierenartsen van voedseldieren aan om naar positieve resultaten te streven.
Percepties van Amerikaanse en Canadese dierenartsen over de kernvaccinatiepercentages van honden en katten en de impact van de anti-vaxx-beweging voor menselijke geneeskunde op de diergeneeskunde
Er werd een elektronische enquête verspreid om de perceptie van Amerikaanse en Canadese dierenartsen over de vaccinatiegraad van honden en katten te beoordelen. De grootste zorgen van dierenartsen bij het vaccineren van een gezonde volwassen hond waren anafylaxie, pijn op de injectieplaats en lethargie; voor katten omvatten deze zorgen vaccin-geassocieerd sarcoom, lethargie en pijn op de injectieplaats. Dierenartsen meldden dat de meest voorkomende zorgen die werden genoemd door onwillige of resistente klanten om hun honden of katten te vaccineren, de overtuiging waren dat vaccinaties duur en onnodig zijn of kunnen leiden tot chronische of ernstige ziekten.
Er was een positieve correlatie tussen een georganiseerde anti-vaxx-beweging tegen verplichte vaccinatie voor kinderen in hun gemeenschap en het aantal vaccinresistente of bezorgde cliënten. Dat het aantal resistente cliënten in verband werd gebracht met de aanwezigheid van een georganiseerde anti-vaxx-beweging, impliceert dat de menselijke anti-vaxx-beweging de mening van eigenaren van gezelschapsdieren over vaccinaties van gezelschapsdieren beïnvloedt. Gegevens van de Census of Veterinarians Survey, beheerd door de AVMA Economics Division in 2016 en 2017, werden geanalyseerd.
De Kolmogorov-Smirnov-test met twee steekproeven werd gebruikt om te bepalen of er een verschil bestond tussen inkomensverdelingen voor mannen en vrouwen op verschillende ervaringsniveaus. Kwantielregressie werd afzonderlijk uitgevoerd voor mannelijke en vrouwelijke respondenten om de directe effecten van individuele factoren op het inkomen te bepalen en om de effecten van praktijkbezit versus niet-eigendom op inkomen te vergelijken. De inkomensverdelingen van mannen en vrouwen waren ongelijk bij lagere ervaringsklassen, maar gelijk bij hogere ervaringsklassen.
